On Anzac Day 2010 three Iroquois helicopters intended to participate in commemorative flypasts. On route, Iroquois Black 2 crashed into hills east of Pukerua Bay, killing three crew members and severely injuring the fourth.
In the years that followed the NZDF carried out various internal processes in an effort to understand the causes of the accident and prevent such a tragedy recurring. A decade on, Michael was tasked with reviewing NZDF’s response and whether the organisation had comprehensively addressed issues of causation, fault and responsibility.
Michael delivered his report to the Minister of Defence and found that NZDF’s processes had been thorough and that there were no outstanding matters that needed to be addressed.
